Please stop by our Bauer Head Start/ Early Head Start School Readiness Academy- at time of application In order to fill out and complete an application for your child, please bring the following documentation: Child's birth certificate Proof of income (your most recent income tax return, or a Texas Workforce 4-quarter Earnings Report, subsequent payroll stubs for the previous3 months) Child's Immunization Record Insurance Card (Private Insurance, CHIPS, Medicaid) Proof of Residency (Uitybl with parent's name and current address) .We are also taking applications for Children with Disabilities/Special Needs (Bring documentation with you) EARLY HEAD ENROLLMENT-ages 6 weeks to 36 months & also enrolling Pregnant mothers.
